Note: Information on form must be complete (i.e. phone numbers, etc) and received by HTA within 48 hours following Date of Incident to merit review by the Grievance and Discipline Committee.

Please state what HTA rule was violated. Remember, the D&G Committee cannot rule on a he said, she said situation or on line calls.
